If the rust is so bad its made a hole in the panel then you'd be much better off replacing the whole panel, if thats not an option then cut the rust part away and cut at least 1/2" into the good surrounding metal and then get a peice of clean metal to make a 'patch' up that fits snug into the hole (put it behind and draw around the edge of the hole then cut the pattern out of the new steel) use a mig welder and just tack the part you've made in place, then go OPPOSITE the first weld and tack it again, try to keep the welds as far apart as possible so it doesn't warp the panel - TAKE YOUR TIME.
if you can sand the surface rust away treat the exposed metal with a 'rust cure' just in case you've missed a bit before painting.
